What Do I Need to Get a PA Driver’s License?
At Tags & Tax, we want to ensure your trip to the PennDOT Driver License Center is successful on the first try. Use this checklist to gather your documents.

Note: All documents must be originals. Photocopies are not accepted by PennDOT.
- Step 1: The Medical Physical (For First-Time Drivers)
- You must download and print Form DL-180 (Non-Commercial Learner’s Permit Application).
- Medical Qualification: A doctor, physician assistant, or nurse practitioner must complete and sign the back of the form. This physical is valid for one year from the exam date. Exception: if you are 16, the exam cannot be done more than 6 months before your 16th birthday.
- Proof of Identity & Legal Presence (Bring ONE)
- Valid U.S. Passport or Passport Card.
- Original U.S. Birth Certificate with a raised seal (issued by a State Bureau of Vital Statistics).
- Certificate of Naturalization or Citizenship.
- Valid Permanent Resident Card (Green Card).
- Non-U.S. Citizens: Valid unexpired passport and unexpired foreign visa with I-94 (refer to PennDOT Pub 195US for specific document codes).
- Social Security Proof
- Your original (not laminated) Social Security Card.
- If you are not eligible for a Social Security number, you must provide an official Social Security ineligibility letter from the SSA.
- *Agent Tip: PennDOT now officially accepts W-2s, 1099s, or pay stubs with your full SSN (must show the full 9 digits) for REAL ID. However, bringing the original card is still the fastest way to avoid system flags..
- Proof of PA Residency (Bring TWO) – Required only for applicants 18 and older
Documents must show your name and physical PA address. At least one must be dated within 90 days:
- Current utility bill (Electric, Gas, Water, etc.).
- W-2 Form or current Pay Stub.
- Lease Agreement or Mortgage document.
- Bank Statement or Credit Card Statement.
- Other acceptable proofs can include PA vehicle registration, auto insurance card, tax records, or mailed documents that have gone through the U.S. Postal Service.
- *Note for Minors: If you are under 18, a parent/guardian with a valid PA ID can provide residency proof for you.
- Name Change Documents (The “Bridge”)
- If your current name does not match your birth certificate or passport, you must “bridge” the gap with certified copies of:
- Marriage Certificate(s).
- Divorce Decree(s).
- Court-Ordered Name Change documents.
- Passports
- *Agent Tip: If you have a valid U.S. Passport in your current legal name, it serves as both proof of identity and proof of name change. You do not need to bring ‘bridge’ documents like marriage certificates.
- If your current name does not match your birth certificate or passport, you must “bridge” the gap with certified copies of:
- Forms for Minors (Under 18)
- Form DL-180TD: Parental Consent Form. This must be notarized if your parent is not accompanying you to the License Center.
- Recommended: bring proof of school enrollment or graduation (e.g., high school ID, report card, diploma, or GED), as some locations may request it.
- Fees & Testing
- Payment: PennDOT centers accept check, money order, or debit/credit cards (Visa, MasterCard, etc.). Cash is NOT accepted at most locations; check your specific center in advance.
- Tests: Be prepared to pass a vision screening and the Knowledge (Permit) test on your first visit.
